Gravity drainage requires the collection bag to be below the bladder so urine flows toward the bag and does not reflux back into the bladder. Keeping the bag lower prevents backflow, which lowers the chance that bacteria move up into the bladder and cause a urinary tract infection. In practice, place the bag on the floor or a lower stand, secure the tubing to prevent pulling or kinking, and never let the bag rise above the level of the bladder. The considerations about leaks, comfort, or concealing the bag are secondary; the core reason is infection prevention through proper gravity drainage.
